Medication AuthorizationClick the link below to download a medication authorization form to be completed by your child's doctor, if your child will be taking medications at school during the coming school year. Medication will only be given with a medication form completed and signed by your child's health care provider and brought to the health office with the medication in a labelled container. Written parent permission to give medication in school is also required. This form must be in school on the first day if you wish us to administer the medication that day. A new order will be needed anytime there is a change in medication or dosage. When filling a medication prescription, ask for an extra labeled bottle for school.Over the counter medications, for example, Tylenol, Aspirin, Ibuprofen may only be given in school with a Health Care provider order and from the original labeled container. Children on Medication for Asthma Please provide us with a peak flow meter along with your child's medication so that we can more closely monitor your child should he/she experience an asthma problem during school. Please consult with your physician regarding your child's baseline reading and guidelines for using the asthma medication and peak flow meter at school. Orders from health care provider are needed for asthma inhalers carried by the student or kept in the health office. Your health care provider needs to indicate on the order that the student is capable of self-administration of the inhaler. It is highly recommended that students who carry their own inhaler leave a back-up inhaler in the health office. Children on Medication for ADD or ADHDPlease consult with your physician regarding having your child's teacher complete behavior checklists to help monitor the effectiveness of the medication. Ask your health care provider to send us the form which they find most helpful to them, so we can assist you in monitoring the effectiveness of the medication.Children on Medication for a Severe Allergy: Please have your physician give us the dose written on the order as well as specific instructions, such as whether to administer the injection immediately or only if reaction symptoms occur. Once the order is received an emergency plan will be written and sent home for your approval. Please provide an epi-pen for school if ordered. If your child needs to carry an epi-pen, then please provide a back-up epi-pen for the health office. |
